The survey only takes a few minutes to complete, open until Sunday, 1 June 2025.

Online survey forms are here: Community or Easy English Survey. There is also a pdf available for download if you, your family or friends would prefer to fill out a hard copy.  If you have ongoing interest, please register on that page to Stay Informed as we progress through our review and developing a new DIAP next financial year.

If you have the capacity to do so, and as necessary, please consider helping people with lived experience to complete the survey.

Hard copy surveys can also be collected at and delivered back to Council’s administration buildings and libraries, or scanned and emailed to council@shoalhaven.nsw.gov.au.

Your feedback will help us understand your experiences in the Shoalhaven, identify ways Council can support people with disability, their carers, families and service providers, and help the community to foster greater understanding and inclusion of people living with disabilities.

The information you provide will be confidential and used only for the purpose of reviewing and developing the Disability Inclusion Action Plans.

View the full documentation and plan updates, or fill in the survey at Council’s website.
